For Educators
Once you have chosen and contacted an artist from our Artists in the 5th Roster, please download and print out the forms below.Please fill them out during your collaboration session with the artist.
Paperwork can be submitted to CAHEP via fax: 577-3781 Att: CAHEP or dropped off at our office (Confederation College, Shuniah Building (main building), Room 261A.
Due to limited funding, paperwork MUST be submitted before your project begins.

If you are interested in participating in Artists in the 5th but cannot locate the Artists in the 5th Artist Roster sent to your school in a package in September, please email Alana at aforslund@confederationc.on.ca for a printable version of the roster.
